Executive Summary
2025 was a pivotal year for the Centene Foundation, which distributed $34.9M in investments to 209 unique nonprofit organizations across 29 states and the District of Columbia. The Foundation expanded its distinctive local approach to giving through several new strategic national partnerships designed to address complex, systemic barriers to community health, including affordable housing, healthcare access, and youth development in underserved communities.
Building on its collaborative model, the Foundation paired financial investment with access to Centene's subject-matter expertise in priority areas such as maternal and child health-strengthening program design while advancing scalable community health outcomes. Looking ahead to 2026 and beyond, the Foundation is positioned to foster knowledge-sharing networks among its nonprofit partners, extending the reach of every dollar invested and advancing an integrated giving strategy that delivers on its mission to transform the health of the communities it serves.

By combining a tailored, local approach with strategic national partnerships, the Foundation has developed a powerful model for giving that addresses both immediate community needs and complex, systemic barriers to health equity.
Our Local Partnerships
In 2025, our disbursements spanned 209 nonprofits in 29 states and the District of Columbia. Key focus areas for the year included affordable housing, social connection, maternal and child health, and healthcare access and quality.
Improving Maternal and Child Health Outcomes in Arkansas
Heartland Forward has established a Maternal and Child Health Center for Policy and Practice to improve maternal and child health across 20 Heartland states.
A three-year, $1M investment by the Centene Foundation and Arkansas Health & Wellness will support a Center initiative focused on using midwifery to cost-effectively improve access and outcomes, particularly in rural areas.

Health Net are contributing
$10M over three years to rehabilitate and expand Encompass Housing's Community Care Campus in Long
Beach. Located in a former hospital, the campus provides an integrated service hub for the most vulnerable populations, addressing critical drivers of health such as housing, workforce development, and behavioral health.
Addressing the Loneliness Epidemic in Northern Indiana
Indiana University Health Foundation's Congregational
Care Network supports senior and other adults who are experiencing
social isolation and need extra support and resources when they
are transitioning from hospitals to homes or care facilities. A two-year, $770K commitment from Centene Foundation and Managed Health Services will help expand the multi-faith network to serve
an additional 12 congregations.
Strengthening Community Care Coordination for Pregnant Individuals in Ohio
The Centene Foundation and Buckeye Health Plan are investing $1.65M over three years to expand the
nationally recognized Pathways Community HUB Institute model
across Ohio, with a focus on preventing preterm births. The HUBs operate as coordinated care networks that identify high-risk individuals, assess their health and social needs and connect them with appropriate services and resources.

Our National Partnerships
Through its partnership with the National Association of Community Health Centers, the Foundation is strengthening Community Health Centers as the nation's primary care safety net by advancing value-based, whole-person care models and improving maternal and child
health outcomes.
The Foundation is working with McCormack Baron Salazar to accelerate the development of affordable housing projects that include onsite social services by providing predevelopment loans at below-market rates.
The Foundation has partnered with Big Brothers Big Sisters of America to support youth development and long-term well-being, expanding mentorship programs that promote social connection and help young people build healthier, more
resilient futures.

Bringing Our Partnerships to Life
Most of the Foundation's commitments are layered, multi-year investments that enable partners to plan, build capacity, and deliver sustained outcomes. Several of our initiatives reached the end of their terms in 2025, with projects that began as transformative ideas now delivering measurable impact in communities across the country.
Community Health Development, Inc., Texas
In July 2025, the Centene Foundation, Superior HealthPlan, and Community Health Development, Inc. (CHDI) celebrated the grand opening of Uvalde's new 16,000-square-foot Multi-Purpose Community Health Center.
First announced in the immediate aftermath of the 2022 Robb Elementary School shooting, the center was made possible through a combined investment of more than $8M from the Foundation and Superior HealthPlan. Providing healthcare services, community outreach, education, and workspaces, the center embodies all three of the Foundation's philanthropic pillars.
CHDI has been recognized by the National Committee for Quality Assurance as a Patient-Centered Medical Home, reflecting their commitment to delivering relationship-based healthcare, with an orientation toward care of the whole person.
Centene CEO Sarah M. London joined representatives from CHDI and Superior HealthPlan for the July 28 ribbon cutting.
CHDI was intentional in designing a center that would bring the community together and offer multiple resources under one roof. In addition to providing healthcare and social services, the building is also home to Uvalde's new Big Brothers Big Sisters of America affiliate.

Feeding Families in Need
In late October 2025, when a federal government shutdown delayed food assistance benefits, the Foundation mounted an immediate response. Within days, the Foundation Board approved
$1.47M in emergency funding, which was disbursed to
71 nonprofits in 5 states.
We have seen a significant increase in requests for food items over the past year, especially recently. This funding will go a long way to provide nutritious food for local, low-income seniors, among the most vulnerable groups in our community.

Project Access, Nebraska
In 2023, Centene Foundation and Nebraska Total Care invested $3M in a three-year partnership with the Health Center Association of Nebraska to establish Project Access, a program to improve patient experience, increase access, and expand network capacity at Federally Qualified Health Centers.
Results:
1,975 patient visits completed in Q2 of 2025 by providers receiving loan repayment or hiring incentives through Project Access

By the end of 2025, five health centers had capital improvements underway
Georgia Southern University, Georgia
In 2023, Centene Foundation and Peach State Health Plan invested $2.2M over two years to address rural health shortfalls in nursing, behavioral health, addiction
counseling, and social work by supporting Georgia Southern University in building a healthcare workforce pipeline.
Results:
3 staff positions created to lead a nursing recruitment and retention program
New addiction recovery program launched in spring 2025
52 students registered in addiction recovery minor courses in spring 2025
GoodLife Innovations, Kansas
In 2024, the Foundation and Sunflower Health Plan made a $200K, one-year investment to help GoodLife Innovations improve care for people with intellectual and developmental disabilities and co-occurring behavioral health needs. The program remotely connects board-certified behavior analysts with rural providers, offering free training and interventions that would otherwise be unavailable or unaffordable.
Results:
186 individuals received targeted training
68 agencies across Kansas were impacted by training or consultation efforts
83% of trainees said they had applied training techniques in their work
Oakwood Creative Care, Arizona
In 2024, the Centene Foundation and Arizona Complete Health invested $2M in a two-year partnership with Oakwood Creative Care to create a new campus for individuals and families navigating the challenges of dementia. October 2025 marked the grand opening of the new facility, which will house:
A first-of-its-kind Adult Day Club for people with mild cognitive impairment
A support and information hub
A soon-to-open wellness clinic featuring age-accessible workout equipment
